110-300-0115(1) Licensor observed an assistant at the daycare that was not in the electronic workforce registry (MERIT) and confirmed by the department. All household members, staff and volunteers must establish a records system in MERIT. 110-300-0115(2) Licensor reviewed staff files and observed them not to be complete. The assistant was missing their file that included their first and last name, date of birth, job title, first and last day of employment, and proof of professional credentials. 110-300-0115(3) Licensor reviewed staff files and observed them not to be complete. The assistant was missing their file that included current government issued photo identification, emergency contact information, and a completed employment application or resume. Provider was missing a copy of their SSN.
